New Patient Registration
We welcome new patients who live within our practice area. The reception team will advise new patients if their home is within our area and patients can also check this on the Policies section of the website - Our Practice Area. All new patients are asked to complete a registration form, medical and alcohol consumption questionnaires and sharing consent form prior to registration at the practice. These are available from our receptionists and can also be downloaded from this page. The practice requests patients wishing to register with us bring along a means of identification, ideally with a photograph on it.
We are only able to accept a registration on receipt of the fully completed registration form, medical and alcohol consumption questionnaires and sharing consent form. Incomplete forms will be returned to patients requesting to register.
New patients can see a GP or Nurse on the day following processing of their complete registration forms.
Patients on repeat medication will need to see a doctor before getting their first prescription. An appointment will be offered as appropriate. We advise patients who wish to register with us to obtain one month's supply of repeat medication from their current GP before registering with Brig Royd Surgery to ensure a smooth handover.
All new patients are welcome to make an appointment within six months of registration.
