Lucy Lawton (f)

Lucy works closely with the GPs managing patient prescription requests and ensuring patient records are updated following any medication changes made by secondary care Consultants and other clinicians involved in the care of our patients.

Lucy's role involves contacting patients who have queries about their prescriptions and reminding patients to attend for blood test monitoring in the line with the prescribing guidelines for some medication.

Social Prescribing Link Workers

Social Prescribing helps you to explore services that may help to improve your health and wellbeing.

Social Prescribing Link Workers work with patients registered with GP’s across Calderdale including both adults and children and young people.

A patient can be referred to a Social Prescribing Link Worker by requesting an appointment themselves through their GP surgery or can also be referred by another service or agency they are already working with.

Social Prescribing Link Workers support patients with a wide range of issues impacting on their health and wellbeing including, but not limited to mental health difficulties, financial difficulties, housing, training and employment, social isolation, drug and alcohol misuse and relationship difficulties including domestic abuse. 

Our Social Prescribing Link Workers are Kelly, Amanda and Eliza.
